General Agreement
By accessing Health Iva Care services, you agree to these Terms governing your relationship regarding healthcare services, website usage, and all interactions. Continued use constitutes acceptance of modifications.
Medical Services
All services require proper intake procedures including completion of forms, provision of accurate health information, and acknowledgment of consent forms. Treatment plans are developed collaboratively between patients and providers.
Payment & Billing
Patients are responsible for co-payments, deductibles, coinsurance, and non-covered services at time of service unless alternative arrangements are approved. Outstanding balances over 90 days may be referred to collections.
Cancellation & No-Shows
Appointments should be cancelled at least 24 hours in advance. Late cancellations or no-shows may incur fees per published schedule. Genuine emergencies are handled compassionately on a case-by-case basis.
Telemedicine
Telemedicine requires appropriate technology, stable internet, and private setting. Certain conditions require in-person evaluation. By participating, you acknowledge potential limitations compared to in-person care.
Liability
Health Iva Care provides services in good faith according to accepted standards. Liability is limited as permitted by law. Disputes are resolved through binding arbitration in Fort Knox, Kentucky.

These terms are governed by laws of the State of Wisconsin and United States federal law. Disputes are resolved in courts of competent jurisdiction in Fort Knox, Kentucky.
